Case Manager Homecare RN\nMust have experience with Certified Home Health Agencies (CHHA)\n\nRESPONSIBILITIES - not limited to : \n- Assesses and charts observations of the patient’s condition at each visit.\n- Completes evaluation tasks, including reviewing medication, vital signs, and \n complete head to toe assessment.\n- Administers skilled nursing treatments and interventions including but not limited to \n at-home IV infusion, wound assessment, wound care, and education aimed at \n promoting and maintaining patient independence.\n- Revises plan of treatments and discharge plan in conjunction with registered nurse \n case manager, patient, family, physician, care delivery team, and third party payors \n including obtaining medical orders and approval of medical services.\n- Initiates appropriate preventive and rehabilitative strategies.\n- Consults with and educates the patient, family and other team members regarding \n disease process, self-care techniques and preventive strategies.\n \n \nQUALIFICATIONS : \n1. New York State Licensed RN\n2. One year of experience as a clinical nurse in a hospital or other direct patient care \n clinical setting required\n3. Strong critical thinking skills, the ability to follow standards / procedures, and make \n appropriate clinical judgments to provide safe and effective client care\n4. Ability to work independently with minimal supervision, adapt to change, and \n manage a variety of assignments while establishing priorities and ensuring the \n necessary duties are completed\n5. Strong interviewing, clinical assessment and communication skills\n6. Ability to exercise initiative and independent judgment\n7. Ability to build and maintain professional relationships\n8. Current NYS driver’s license
