We are seeking a compassionate and skilled Nurse Practitioner (NP) to join our team!
Responsibilities :
Provide comprehensive primary care services to patients, including :
Annual wellness exams
Sick visits
Chronic disease management (diabetes, hypertension, etc.)
Preventive health screenings
Medication management
Weight loss and functional wellness
Collaborate with physicians and other healthcare professionals to deliver integrated and patient-centered care.
Build strong relationships with patients, providing education and support to empower them in their health journey.
Maintain accurate and detailed patient records using our electronic health record (EHR) system.
Adhere to all state and federal regulations and practice guidelines.
Qualifications :
Active Virginia Nurse Practitioner license.
National certification as a Nurse Practitioner.
Active Medicare provider ID.
At least three years of experience as an NP in a primary care setting is preferred.
Experience in geriatrics or internal medicine is a plus.
Excellent communication, interpersonal, and patient care skills.
Strong commitment to providing holistic and patient-centered care.
