Position Overview:
As a Case Manager with TBAH, your role is vital in facilitating and overseeing the smooth transition of returning citizens from incarceration back into the community. Your skills in relationship building, goal setting, and comprehensive oversight will significantly impact and contribute to the success and progression of program participants through their reentry journey.
Key Responsibilities:
- Develop, nurture, and maintain relationships with participants, understanding their unique needs, challenges, and aspirations.
- Craft individualized reentry and employment plans, setting realistic and achievable goals with participants.
- Conduct prison visits to initiate early rapport-building and planning with soon-to-be returning citizens.
- Perform house visits to assess living conditions, provide ongoing support, and ensure that reentry plans are on track.
- Navigate through various communities across Hillsborough, Polk, and Pasco counties to meet with participants and associated entities.
- Work closely with external agencies, community partners, and internal teams to facilitate holistic support for participants.
- Oversee and guide participants through their reentry process, ensuring they have access to necessary resources, support, and opportunities.
- Maintain meticulous records of participant progress, challenges, and outcomes.
- Engage in problem-solving and crisis management, providing empathetic and practical support to participants in times of need.
Qualifications:
- Bachelor’s degree in Social Work, Psychology, Criminal Justice, or a related field.
- Proven experience as a Case Manager, preferably within a context related to returning citizens or similar populations.
- Ability and willingness to conduct visits to correctional facilities and participant residences.
- Strong interpersonal and relationship-building skills, with a capacity to engage with individuals from varied backgrounds.
- Robust organizational and documentation skills, with an ability to manage multiple cases efficiently.
- A valid driver’s license and willingness to travel locally as and when required.
Preferred Qualities:
- A profound understanding of, or experience with, the challenges faced by returning citizens during reintegration.
- Motivational Interviewing (MI) credential.
