What are the responsibilities and job description for the Health and Wellness Coordinator - Sulphur Springs YMCA position at TAMPA METROPOLITAN AREA YMCA INC?
Under the direct supervision of the Community School Director, the Health and Wellness Coordinator will provide direct leadership to the Community Partnership School health and wellness programs. The Coordinator will work with the Community School Director and school administration to identify student, and family needs to implement and facilitate health and wellness programs at the school. The Coordinator will manage the Paw Pantry operation, and assist with family HUB services to ensure families have access to, and are matched to the resources available to them.
Critical areas of expertise include; cultivating partnerships, managing program quality, community outreach, marketing, volunteerism, and relationship building.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
Program Quality and Growth
- Health & Wellness Program > Serve as the primary collaborator between the administration building, Community Partnership School and health partners.
- Supervise and coordinate all health partner referrals, including health, dental, vision and mental health referrals.
- Establish partnerships with health providers to deliver educational programs focused on nutrition, physical activity, mental health awareness, and preventive care, aiming to foster lifelong healthy habits among students.
- Collect data from Health and Wellness Partners that align with grant metrics and deliverables. This may include regular health assessments, student health data, community referrals, dental, vision, and mental health referrals.
- Act as a wellness champion within the school community, encouraging involvement in wellness programs and promoting a culture of health and well-being.
- Evaluate the effectiveness of health programs and services, seeking feedback from students, parents, and faculty to make informed enhancements.
- Intervention Team > Participate in Student Services team meetings to provide information on behavioral interventions and relevant referrals as needed.
- Track and follow up on all referrals made in and out of school.
- Establish referral network/linkages with outside agencies.
- Utilize data management system for proper documentation.
- Participate in the quality improvement process.
- Assist in developing additional procedures and training materials for staff.
- Assist with special projects as needed.
- Operations/Team Leadership
- Adhere to and ensure enforcement of program expectations, and communicate them positively and effectively to staff, members, and program participants.
- Participate in Community Partnership School team meetings, traditional school meetings, Tampa YMCA meetings, and collaborative partner meetings as appropriate.
- Manage projects as assigned and coordinates all participants, supplies and scheduling necessary to complete them.
- Create and maintain meaningful and impactful relationships with families, school administration and staff, and community leaders and members
- Ensure staff members and volunteers are scheduled in accordance with safety and program needs and ensures all safety and program ratios are followed.
- Provide oversight to volunteers, including enforcing policy and compliance, training, scheduling, and stewardship.
- Complete all other duties as assigned.
Safety and Risk Management
- Ensure all Tampa Y program standards and best practices are implemented and followed. Applies YMCA policies and procedures, including those related to best practices, emergency procedures, medical and disciplinary situations, and child abuse prevention.
- Use discretion and independent judgment in handling confidential and sensitive information in connection with the Tampa Y program responsibilities and communication to/from leadership, associates, vendors, and guests.
- Assist in all areas, project teams, job tasks as assigned.
- Attend all conferences, trainings, meetings, and presentations as assigned.
