Job Summary
Job Summary: Job incumbent will provide the following supported services for family caregivers of elders age sixty (60) and older and grandparents taking care of children under age eighteen (18); Information about available services through community meetings, health fairs and newsletters, provides assistance in gaining access to services, respite for the caregiver, individual counseling, organization of support group and educational training.

Essential Functions

Representative Duties: Under general supervision of the Homecare Manager job incumbent will:
1. Assist with patient care coordination. Assisting Care Coordinator with documentation of patient services including referral, support system, and specific medical or physical need.
2. Implementing Home and Community based waivers.
3. Complete assessments, develop and revise patient centered written care plans. Review plans of care.
4. Communicate with Home Care Provider, patient, family, Health Aide, health professional, Tribal Council, and others regarding patient health care needs.
5. Provides CPR, first aid and continuing education units.
6. Assist with implementation of program activities as directed. Assist with program needs analysis. Perform consumer satisfaction surveys.
7. Attend and participate in meetings and online based teleconferences as required.
8. Must maintain strict confidentiality in accordance with HIPAA standards.
9. Communicate with family caregiver, elder, and village provider for supportive services.
10. Provide and develop educational material to support caregivers through one on one contact, health fairs, schools and community meetings.
11. Update and maintain caregiver database with current information to include: name, date of birth, address, telephone number and services received.
13. Assess home care patients and caregivers annually for eligibility of home care services.
14. Compile monthly and quarterly reports for program director.
15. Refer, provide counseling or support group activities for caregivers.

Minimum Qualifications
Minimum Qualifications:
1. Associates degree in social work, psychology, rehabilitation, nursing or other closely related human services field. Bachelor of Arts or Science preferred.
2. Certified in basic life support.
3. Two years’ experience in Home Care long term care of delivery in community based services.
4. This position requires state certification in Home and Community based waiver program.
5. Previous experience working with Alaska Natives or other cross-cultural experience.
6. Must pass background check pursuant to federal Indian Child Protection and Family Violence Prevention Act requirement and State of Alaska background check.
7. Previous work experience working with the elderly.
Knowledge, Skills and Abilities:
1. Strong verbal, written, interpersonal, and organizational skills.
2. Ability to manage multiple projects, prioritize tasks, and ability to work independently.
3. Proficiency using MS Office applications.
4. Ability to maintain strict confidentiality.

Supervision
Physical demands: Frequent travel to interior villages required. Incumbent may be expected to work extended hours. Conditions include but are not limited to: travel by small un-pressurized aircraft, poor heat, lack of running water, and adverse weather conditions. Position requires bending, stooping, kneeling, reaching, stretching and a considerable amount of walking. Job incumbent may be required at times to lift up to thirty pounds in support of equipment and supplies. Incumbent must be able to work accurately in a busy and sometimes stressful environment.

Summary: Position requires strong leadership and management skills and the ability to communicate effectively with other members of the care team. Position requires the ability to adapt and communicate effectively with clients, home care providers and tribal administrators.
