Job Summary



Job Summary: The Special Diabetes Program for Indians (SDPI) RN Diabetes Care and Education Specialist (DCES) provides diabetes self-management education and support (DSMES) for patients with pre-diabetes, diabetes, and gestational diabetes. The RN DCES provides direct patient care within the RN scope of practice including DSMES, case management, care coordination, and foot care.

Adhere to the TCC Ch’eghwtsen’ model of service and guiding principle which requires providing timely and effective service along with the ability to interact with others in a way that inspires trust and demonstrates respect, compassion and empathy.


Essential Functions

Representative Duties:
Under the direct supervision of the Diabetes Program Manager, job incumbent will be assigned from the following representative duties:

1. Integrates skills and knowledge of pathophysiology, epidemiology, clinical management, cardiometabolic conditions, and self-management of diabetes into clinical practice.

2. Applies the ADCES 7 self-care behaviors (or other program approved DSMES curriculum) to educate and facilitate behavior change.

3. Provides quality DSMES in individual and group settings based on assessed needs. Utilizes appropriate teaching techniques that are sensitive to the learning preferences of the person with prediabetes or diabetes.

4. Completes comprehensive assessments for each patient including emotional and behavioral health, interprets personal health data, develops an individualized care plan based on the patient’s assessed needs and goals and promotes successful self-management.

5. Documents all individual contacts/visits in the Electronic Health Record and outcomes data based according to the guidelines in a timely manner.

6. Collaborates, advocates, and confers other members of the diabetes care team in developing person-centered diabetes plans.

7. Advocates for and supports technology-enabled diabetes education and care, in individual and population health diabetes services.

8. Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards. Complies with TCC, government and accreditation regulations.

9. Identifies and actively participates in the quality improvement (QI) process and adapts practice/process based on QI findings.

10. Coordinates patient appointments for annual physical examinations and preventive services for patients with diabetes.

11. Conducts annual diabetes chart audit and performs focused audits of indicators of concern as identified by recurrent patterns of occurrence or as directed by the Diabetes Program Manager, TCC Leadership and/or the Quality Management Committee. Also participates in annual grant preparation under direction of program manager.

12. Travels to villages for the purpose of providing DMSES, community education, and foot care clinics.

13. Complies with CAIHC infection control and hazardous materials exposure control policies.

Other Responsibilities:

1. Operates medical office equipment.

2. Participates in Community Health Nursing and CAIHC QM studies.

3. Perform other job-related duties as assigned.


Minimum Qualifications



Minimum Qualifications:

1. Graduate of an accredited school of nursing is required; Bachelor of Science in Nursing preferred.

2. Two years of experience working as an RN in either inpatient nursing or ambulatory care with demonstrated experience providing care for diabetes patients and strong knowledge of the diabetes disease process. Experience in diabetic assessment, care planning, and patient education.

3. Licensed as a Registered Nurse in the State of Alaska required.

4. Certified or eligible to be certified as a Diabetes Care and Educator Specialist (CDCES) or Advanced Diabetes Management (BC-ADM) preferred. If not certified, willing to work towards certification and obtain when qualified.

5. Previous experience working in rural Alaska or with providing health care to Alaska Natives preferred.

6. Alaska Driver’s License is required.

7. CPR certification is required.

8. Must pass background check pursuant to federal Indian Child Protection and Family Violence Prevention Act requirements.

Knowledge, Skills and Abilities:

1. In-depth knowledge of diabetes patho-physiology and treatment strategies preferred.

2. Good verbal and written communication skills.

3. Knowledgeable and experienced in working with various types of diabetes technology preferred.

4. Organized and attention to detail.

5. Able to adapt curriculum content to varying cognitive abilities, beliefs, and cultures.

6. Problem solving and critical thinking skills.

7. Ability to collect accurate data, maintain records, and compile reports.

8. Engages in lifelong learning and serves as a role model of professionalism.

9. Computer literacy including use of Microsoft Office.

Supervision:
This position does not have supervisory responsibilities.


Supervision



Physical demands: Work requires walking, standing, and occasional lifting, bending, and pushing to assist patients, move equipment, and issue supplies. Sedentary activities consist of sitting at a desk, typing, and computer operation. Knowledge and use of proper body mechanics and office ergonomics is required. Some degree of risk is associated with driving during winter conditions with arctic temperatures. Travel to villages in small aircraft is required.

Work environment:
There is common exposure to contagious disease. Some degree of risk is also present in working with alcohol-affected individuals and emotionally disturbed patients.

Occupational Exposure to Other Potentially Infectious Materials:
Occasionally exposed to blood, body tissue, and other potentially infectious materials; Exposure Category I.

Summation: Patients served are primarily Alaska Natives. Must be patient and willing to effectively teach educational materials to patients at different levels of learning.
