Job Summary: The Social Services Coordinator provides assistance to patients in conjunction with the medical staff by utilizing services within Tanana Chiefs Conference (TCC) Health Division and resources in the community. Incumbent provides resources to patients that are identified by utilizing a needs assessment questionnaire identifying barriers to their care and to help problem solve solutions, creating stability in their lives, in an effort to support the achievement of the goals of their medical plan of care. This position works in a multi-disciplinary team-based system of care providing services for specific regions and villages served by TCC. The incumbent is a member of the primary care team and serves as a liaison between patients, family/caregiver, primary care provider and other clinical staff. Communication between all members of a patient’s care team is essential. Supporting our patients and family/caregivers to ensure identified social needs are addressed and timely coordination of resources is received. This affords the job incumbent with the opportunity to engage with patients from across the TCC region and have a positive impact on the patient experience and health outcomes.

Essential Functions

Representative Duties: Under the direct supervision of the Social Services Manager the job incumbent will:
1. Provides social services coordination based on referrals from medical providers and other sources. Responds by connecting patients with available resources to meet the identified need both in Fairbanks and throughout the TCC region.
2. Identifies, tracks, and follows up with patients who need social support in order to engage in their health care and achieve the goals of their medical plan of care. Maintains awareness of the current status of referrals of care in order to direct and assist the primary care team.
3. Assists the patient with navigating the available systems of services to meet their identified needs. Provides direct support for patients who need help completing applications for assistance and enrolling in available social service programs.
4. Ensures closed-loop communication with clinical staff and referring providers so that all members of the patient’s care team are informed and aware of the services the patient is receiving to address identified barriers to care.
5. Closes the loop with providers and staff on social services referrals to ensure that both the patient and their PCP receive results in a timely fashion, allowing the highest quality of care and the achievement of the desired health outcomes.
6. Frequent communication by phone, electronically, and in person with patients, medical providers, nursing staff, and other support staff. Manages the receipt and transmission of protected health information (PHI) as needed to facilitate care.
7. Willingly accepts and performs other job-related duties as assigned or required.
8. May be asked to travel to communities within the TCC region to provide coordination of services directly to patients.

Minimum Qualifications
Minimum Qualifications: Incumbents must meet minimum qualifications for this position with the following combinations of education, training, and experience:
1. High School diploma or GED required.
2. Preference for experience or training working directly with clients in a social service or health care related field.
3. Familiarity with medical terminology and medical procedures.
4. Proficiency with PC-based office productivity software (Microsoft Office, Outlook, etc.) and ability to operate standard office equipment.
5. Ability to maintain patient confidentiality, interact effectively with patients and fellow staff and communicate clearly. Excellent verbal and written communication skills.
6. Strong critical thinking and decision-making ability.
7. Must be dependable, prompt, motivated, and able to work under minimum supervision.
8. Strong affinity for team-based work and collaborative problem-solving.
9. Must pass background check under the State of Alaska background check and federal Indian Child Protection and Family Violence Prevention Act requirements.
Knowledge, Skills and Abilities:
1. Strong computer skills; i.e. electronic records, publisher, excel, word-processing software programs and e-mail.
2. Strong verbal, written, and interpersonal skills to problem solve dilemmas and crisis situations.
3. Ability to relate well to individual diagnosed with a serious mental illness.
4. Knowledge and ability to adhere to Health Insurance Portability and Accountability Act (HIPAA) and 42 CFR Part 2 confidentiality regulations.
5. Knowledge of Medicaid third Party Billing preferred.

Supervision
Physical demands: Position is mostly sedentary with extended periods sitting at desk/computer. Job incumbent must be able to lift and carry up to 50 pounds. Incumbent must be able to work efficiently in a busy and sometimes stressful environment. Shared office space in a clinical setting.
Work environment: Work is performed in a clinic responsible for treating patients with a wide variety of medical problems. The employee may be exposed to communicable disease during performance of duties.
