What are the responsibilities and job description for the Behavioral Health Counselor (School-Based) position at Tandem Health?
Duties and Responsibilities:
- Completes mental health assessments and treatment plans that identify service needs and recommends options for care.
- Formulates programs for treatment of patients using knowledge of current mental health diagnosis and counseling and treatment techniques.
- Counsels patients individually and/or in group sessions (as appropriate) to assist the patients in overcoming individual or family issues.
- Prepares and maintains reports and case histories.
- Monitors the condition of the client to evaluate success of therapy, and adapts treatment as needed.
- Consults with students' educational teams to establish and monitor specific, evidence-based strategies for use in the classroom from the perspective of a behavioral health provider.
- Counsels family members (as appropriate) to assist with treatment issues and to serve as a support system to the patient.
- Conduct dangerousness assessments (risk to harm of self and others) and collaboration with outside crisis agencies as needed.
- Assists in the coordination of services with other disciplines
- Refers patients to specialty services when necessary and advocates for patients in arrangement of services
- Remains current and knowledgeable about local and regional resources for mental health and substance abuse assessment, intervention, and treatment for children, adolescents, and families
- Provides case management services to assist patients and their families with access to needed benefits and services. Refers patients to other supportive services, as needed.
- Provides education and outreach activities to school and community as appropriate.
- Participates in activities necessary to implement the multi-disciplinary patient-centered medical home approach for the care of patients
- Participates in quality assurance processes to assure optimal care delivery
- Performs other duties as assigned.
Qualifications:
- Master's Degree in Counseling, Social Work, or related field required. LISW, LPC, LMFT or Psychologist licensed with the South Carolina Labor Licensing Regulation required.
- One to two (1-2) years of counseling experience preferred.
- CPR certification required.
