A health services organization in New York City is currently seeking a licensed Registered Nurse (RN) to join their growing team in Brooklyn as their new Coordinator of Care. In this role, Coordinator of Care (RN) will assume the ongoing, primary responsibility to coordinate, implement and continually evaluate the home care needs of patients.
About the Opportunity :
Schedule : Monday to Friday
Specialty : Home Care
Patient Population : Adults
Patient Volume : 6 to 7 per day Responsibilities :
The Coordinator of Care (RN) will :
Review patient insurance information
Coordinate activities involved in each individual care plan
Make sure that pertinent findings are shared with family and professional caregivers
Perform other duties, as needed Qualifications :
1 year of experience in a Medical-Surgical, Hospital, and / or Home Care role / setting
Graduate of an accredited School of Nursing
NYS Registered Nurse (RN) license
Solid assessment and documentation skills
Patient oriented
Great interpersonal skills
Excellent communication skills (written and verbal)
Strong attention to detail
Highly organized Desired Skills :
Bachelor's Degree
Bilingual
