A health services network on Long Island is currently seeking a new Licensed Practical Nurse (LPN) to join their growing team at one of their facilities in Nassau County.
About the Opportunity :
Start Date : ASAP
Assignment Length : Ongoing
Setting : Residential ICF
Shifts : Evenings
Hours : 3p-11pm (Monday, Wednesday & Friday)
Responsibilities :
The Licensed Practical Nurse (LPN) will :
Assist in data collection on individuals within assigned caseload
Contribute to the development of individual's plans and assist in identifying areas of need.
Assist in observation and evaluation of nursing care administered by Direct Support Professionals
Ensure that all required documentation is completed / processed appropriately and in a timely manner
Assist in maintaining effective communication with staff, physicians and Registered Nurse.
Review physician summary forms to ensure information is complete, accurate, and documented in the individual's medical record
Ensure Emergency Room instructions and / or treatment is followed up and documented accordingly
Attend medical appointments when necessary and / or requested by the Supervisor
Assist with ensuring accuracy of MARs for use at beginning of the month
Ensure all individual's self-preservation through implementation of the fire evacuation plan when participating in scheduled fire drills
Perform other duties, as assigned
Qualifications :
Graduate of an accredited Practical Nursing program
NYS Licensed Practical Nurse (LPN)
Solid assessment, clinical, and documentation skills
Patient oriented
Great interpersonal skills
Excellent communication skills (written and verbal)
Strong attention to detail
Highly organized
Desired Skills :
Bachelor's Degree
Experience working with the differently-abled population
