A HighTech Medicare Advantage Care Company is seeking a compassionate Primary Care NP specializing in family, internal, or geriatric medicine to join their In-Home Care team. Based in Nothern New Jersey, this role involves providing care to vulnerable patients with chronic conditions, frailty, and advanced illness, through both in-home.
Responsibilities:
The Primary Care NP will be responsible for:
- Delivering primary medical care to assigned patients in their homes to support their Medicare Advantage Care Plan!
- Creating patient-centered care plans based on their values and needs.
- Evaluating prognoses and structuring care plans, including advanced care planning.
- Leading interdisciplinary team (IDT) meetings to ensure patient care plans are executed effectively.
Qualifications:
- ANP, FNP or GNP
- Experience with: Home Care, Primary Care, Geriatrics, MedSurg, or ICU/CCU

Salary : $140,000 - $160,000
