- Manage and oversee a team of healthcare providers
- Delegate responsibility appropriately and supervise assigned personnel in collaboration with the respective Clinical Manager, in a manner that will assure quality care and compliance with the care plan.
- Assess and evaluate the health care needs of participants, considering physical, mental, and environmental factors.
- Supervise, instruct, and guide Nurses and Aides in the delivery of quality home health care services in the community.
- Conduct joint visits with Nurses to provide supervision and competencies.
- Supervise, instruct, and guide Nurses, Personal Care Aides, or Home Health Aides.
- Assist the Training department in designing the orientation for new employees.
- Acts as a resource for Nurses and team. Conduct field visits in lieu of Nurses as needed.
- Minimum of one (1 ) year of experience in home care as a community health nurse in a Certified Home Health Agency, Long Term Home Health Care Program, and/or Licensed Agency.
- Minimum of one (1 ) year of experience working with a frail or elderly population or, if the individual has less than one (1) year of experience but meets all other requirements, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
- Experience in a PACE Program or MLTC preferred.
