Patient Population : Adults & Geriatrics Responsibilities :
The Care Manager (RN) will :
Provide case management to members, including medical, social, psychological, and physical.
Develop, implement, and monitor the care plan to help meet the member's needs
Perform other duties, as needed Qualifications :
2 years of Case Management and / or Home Care experience
Graduate of an accredited School of Nursing
NYS Registered Nurse (RN) license Desired Skills :
Bachelor's Degree in Nursing
Bilingual (English and Cantonese, Mandarin, or Spanish)
