A recognized healthcare network in New York is looking to add a new Licensed Clinical Social Worker (LCSW) and/or Licensed Master Social Worker (LMSW) to their growing team in the Greater Syracuse area.
About the Opportunity:
- Start Date: ASAP
- Assignment Length: 13-week contract (possibility of extension)
- Setting: Acute Care
Responsibilities:
- Provide comprehensive psycho-social assessment, intervention, management, support, and assess for discharge planning and aftercare needs of the at-risk patient population and their families
- Addresses social determinants of health and works collaboratively with the multidisciplinary team
- Provide discharge planning to patients leaving the hospital
- Perform other duties, as needed
Qualifications:
- Bachelor's Degree
- Active NYS Licensed Clinical Social Worker (LCSW) and/or Licensed Master Social Worker (LMSW)
Desired Skills:
- 3 years of experience in a Hospital and/or Healthcare setting
- Master's Degree
- Bilingual
