Primary Care Provider - In-Home Care
A compassionate primary care provider specializing in family, internal, or geriatric medicine is sought to join our team based in Central New Jersey.
This role involves providing care to vulnerable patients with chronic conditions, frailty, and advanced illness through both in-home and patient-centered care plans.
Responsibilities:
- Delivering Primary Medical Care: Provide primary medical care to assigned patients in their homes to support their Medicare Advantage Care Plan.
- Patient-Centered Care Plans: Create patient-centered care plans based on the patient's values and needs.
- Evaluating Prognoses: Evaluate prognoses and structure care plans, including advanced care planning.
- Interdisciplinary Team Leadership: Lead interdisciplinary team (IDT) meetings to ensure patient care plans are executed effectively.
Qualifications:
- MD/DO
- Licensed in the state of New Jersey
- Board Certified/Eligible in Family, Internal, or Geriatric Medicine
