A New York-based healthcare organization is actively seeking a licensed Physical Therapist (PT) to join their growing team at one of their Rochester-based facilities.
About the Opportunity:
- Shift: Days
- Schedule: Monday to Friday
- Hours: 8am to 4:30pm
- Setting: Inpatient Mental Health
- Patient Population: Chronically and persistently mentally ill Adults (Ages 18 )
- Patient Volume: 25 to 29 per ward)
- EMR: MAARS
Responsibilities:
- Plan and conduct medically prescribed treatment of patients with physical disabilities in a group or 1:1 setting
- Acts as a team member who coordinates physical therapy services with other disciplines
- Attend morning report, and treatment plan meeting
- Attend Barriers to Discharge meetings, and Fall Prevention Committee
- Uphold documentation standards and meeting these requirements in a timely manner including: screening, evaluation, treatment team addendums, progress notes, discharge summaries, ISR comments, etc.
Qualifications:
- 1 year of Physical Therapy experience
- NYS Physical Therapist (PT) license
Desired Skills:
- Bilingual
- Experience with providing Geriatric Care
- Experience with PT Disabilities
- Experience with Fall Prevention
Salary : $1,640 - $2,120
