What are the responsibilities and job description for the RN Case Manager position at Tandym?
Full-time inpatient RN Case Manager role with a top hospital in Fairfield county, CT, just over the border from NY.
The schedule is Monday through Friday, 8am to 4pm, with 2 weekends per month plus holiday rotation required.
The RN Case Manager is responsible for overseeing the implementation of patients' healthcare services.
Responsibilities:
- Facilitates the care of patients to ensure quality outcomes and appropriate utilization of health care resources.
- Collaborate with a multi-disciplinary team in the planning, implementation and evaluation of medical and nursing care
- Provide care coordination in order to close gaps in care
- Performs all job duties and responsibilities in a courteous and customer-focused manner
Requirements:
- Registered Nurse license
- 5 years of strong clinical experience in an acute care setting.
- Additional experience in home care, long term care and case management is preferred
