Area of Responsibilities
Responsible for completion of administrative tasks involved in the placement of patients including setting up patient transport, pre-registration, disseminating necessary information to Willowbrooke departments, reviewing legal paperwork, noting in patient chart, arranging transportation, and ensuring clinical information accessible to attending Doctor. Responsible for completion of tasks in a timely fashion.Responsible for coordination, communication, and documentation of assigned patients pending at outside facilities. The employee will actively pursue placement for patients requiring inpatient treatment, utilizing phone, BHL board, and Trace to ensure outside facilities have the necessary information to determine disposition. The employee will play an active role in locating suitable treatment facilities and diligently following up at least once every three hours with both the referring hospital and outside facility.Responsible for maintaining a working knowledge of department tools and systems. Maintains access and working knowledge to necessary programs including, but not limited to Meditech and Midas. The employee utilizes programs according to policy and completes tasks within designated programs according to work flow.Responsible for obtaining and communicating information to Assessment Services regarding number of proposed discharges and available beds each shift for hospital and all contracted hospitals. The employee will continue to monitor bed status throughout the shift and update the team accordingly to ensure accurate bed count is available at all times.Responsible for obtaining and reviewing external referrals from Tracker fax system and State Contracted Bed Board. The employee will thoroughly review referrals received and make determinations on potential suitability for inpatient treatment factoring in insurance type benefits, medical history, patient presentation, and clinical information. The employee will staff all suitable referrals with Intake Nurse Masters level clinician for review and follow up with the referring facility to feedback on the referral outcome.Compliance Statement
