Coordinates with physicians and primary nurse to communicate to the patient and family the plan of care. Maintains effective communication with nursing leaders. Effectively handles situations in a calm, objective manner and coordinates needed change. Collaborates with and supports other nursing leaders, unit nurses, and the hub. Leads unit care progression rounds to identify barriers to discharge and aligns care team towards discharge goals. Identifies, documents, and escalates barriers to patient progression, as appropriate. Attends escalation rounds to escalate any barriers to progression that require administrative assistance. Communicates service event prioritization and patient preparedness issues to Service
- EFFICIENCY, STANDARDS AND COMPLIANCE: Knowledgeable regarding hospital policy and procedures, regulatory standards, regulatory standards, evidence-based practice guidelines (EMTALA, TJC, CMS, Georgia Board of Nursing, efficiency metrics, etc.) Knowledgeable in the review of patient information within the electronic medical record. Assists director and managers in the identification, coordination and implementation and monitoring of unit specific process improvement opportunities which improve efficiency, minimize errors, improve patient safety and satisfaction, maximize resource utilization and provide a safe work environment. Demonstrates knowledge of own role in patient safety, recognizing unsafe practices. Coordinates patient care activities in collaboration with the hub to ensure appropriate patient placement and level of care. Works collaboratively with unit staff, ancillary services, and support team (CDI, Utilization review and case management) to progress each patient's care towards discharge while assuring quality standards and proactively identifying barriers to discharge. Evaluates discharge needs upon admission, ensuring each patient has a complete discharge plan. refers discharge planning to appropriate role as needed.
- INTERDISCIPLINARY COLLABORATION: Committed to finding solutions to problems rather than blaming others. Demonstrates knowledge of rules, regulations, policies, procedures, standards of nursing practice. Efficiently plans and manages patient progression through the system. Serves as a role model for excellent patient care. Works with primary nurse and case manager on discharge needs barriers for all patients. initiates intervention by UM nurse as needed. Collaborates with UM Nurse to assure that all payor and status requirements are aligned according to patient's clinical progression. Assist Clinical Documentation Specialist in engaging provider to respond to queries clarifying documentation to reflect appropriate severity of illness. Reviews LOS values and working DRG daily and partners with CDI or UM nurses as needed.
- PATIENT PROGRESSION: Ensures that the Care Compass Technology is utilized to update patient progression and barriers to discharge. Coordinates with the nursing units to ensure that all patient updates are charted within the EHR to reflect in the patients care. Collaborates with the hub to ensure appropriate patient placement and level of care (ICU, Med Surg, etc.) Proactively monitors patients in observation or outpatient status to ensure timely discharge or status change consideration. Identifies any unit barriers that may impede department or system-wide patient flow. Collaborates with physicians to ensure patient progression and discharge needs are identified and a plan is developed to address barriers. Identifies potential discharges and updates patient status and planned discharge time. Provides the Hub with visibility to potential unit transfers and notifies in
- EDUCATION AND LEADERSHIP: Assists in provision of education around patient progression. Consults and educates medical and nursing staff regarding status, placement, payor requirements, and utilization of resources. Identifies opportunities to transition patient care to an outpatient setting. Works with unit staff to ensure technology accurately reflects unit and patient status (anticipated transfers, discharges, and patient location). Ensures acknowledgement and completion of Pre-Procedure Requirements (PPRs) prior to testing and or procedures. Ensures paperwork for outbound transfers is completed correctly and sent in a timely manner.
