Essential Duties and Responsibilities
- Collaborate with interdisciplinary team to provide integrated care and manage case management referrals for the behavioral health team.
- Connect patients to social services and community programs, providing advocacy and support as they navigate these systems.
- Conduct outreach to patients lost to care, encouraging re-engagement with services and facilitating access to necessary resources.
- Act as a liaison between patients, staff, and external organizations to strengthen community partnerships and ensure patients have access to current resources.
- Attend community events to build relationships with partner organizations and manage updates to community resource information.
