The Behavioral Health Consultant (BHC) is a key member of the integrated care team, responsible for providing short-term, evidence-based behavioral health services. The BHC will work with patients of all ages in a fast-paced healthcare environment, providing services individually or in groups, in person or via telehealth. You will be responsible for mental health screenings, brief interventions, and consultations to address both mental and physical health conditions in collaboration with medical providers.
Essential Duties and Responsibilities:
- Provide direct patient care, conducting brief targeted interventions, short-term counseling, and screenings (typically 30-minute sessions) to address both acute and chronic behavioral health needs.
- Work collaboratively with an interdisciplinary care team, providing timely feedback and treatment recommendations for patients to their primary care providers.
- Support patients with chronic conditions by using behavioral health principles to address lifestyle changes, health risk factors, and co-management of chronic diseases.
- Ensure care continuity by seeing patients immediately after their medical appointments through warm hand-offs.
- Document patient interactions in the Electronic Health Record (EHR) system, maintaining accurate ICD and CPT coding.

- Master’s degree in clinical social work, counseling, or clinical psychology.
- Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC) in Illinois is required.
- A minimum of three years of clinical experience in a hospital, community health, or mental health setting providing individual and group counseling.
- FOR MAT BHC – Three years of experience working with substance use populations required, including some experience with a MAT program. Additional CADC (Certified Alcohol and Drug Counselor) preferred.
Salary and Benefits:
