The Coordinator, Behavioral Health plays a pivotal role in connecting patients to the care and resources they need. This position focuses on advocating for patients, addressing barriers to services, and ensuring access to critical social and community-based resources. The Coordinator works closely with Tapestry 360 Health’s interdisciplinary team to support holistic, integrated care delivery.
Essential Duties and Responsibilities:
- Advocate for patients navigating the healthcare system, fostering trust and empowerment in patients and their families.
- Act as a bridge between patients, Tapestry 360 staff, and external organizations, ensuring seamless access to community resources.
- Partner with interdisciplinary teams to manage behavioral health referrals and deliver integrated care.
- Conduct patient outreach, re-engaging individuals who have fallen out of care and connecting them to needed support.

Education and Experience:
- Bachelor’s degree in social work, public health, psychology, or a related field required.
- At least 2 years of experience in care coordination, social work, or community health preferred.
Skills and Competencies:
- Exceptional interpersonal skills with the ability to connect with diverse populations.
- Understanding of social determinants of health and their impact on patient outcomes.
- Experience with electronic medical records and a high level of technology proficiency.
- Ability to work independently while contributing to a collaborative, team-based environment.
- Strong organizational skills with the ability to manage multiple priorities effectively.

- Lived experience navigating barriers to healthcare is valued.
- Availability to work flexible hours, including some evenings and weekends.
- Reliable transportation for travel between locations (approximately 20% of the time).
Salary and Benefits:
- Compensation: The annual salary range for this position is between $40,000 and $60,000 based on experience and qualifications.
- Benefits: Tapestry 360 Health offers a comprehensive benefits package including health insurance, dental insurance, retirement savings plans, paid time off, continuing education support and eligibility for the Public Service Loan Forgiveness (PSLF) program.
