What are the responsibilities and job description for the Hospice Aide - CNA position at Tapestry Hospice?
Tapestry Hospice & Palliative Care is family owned and operated – not a big corporation. We have been serving North Georgia for over 16 years and continue to grow and reach new communities. We operate as a Care Family with each other and our patients. The promise to treat everyone like they matter flows over to the workplace where our patients are treated with dignity and compassion.
We are looking for exceptional people to join our growing family. Do you want to feel appreciated and be part of a dedicated team that strives for compassion, comfort and peace for their patients and their families?
As a CNA you are one of the most vital components to the Interdisciplinary Team! You are the heart, the eyes, and the key communicator of your patient’s needs. Your role is to follow a carefully planned out, personalized Care Plan for each patient you are assigned. Report and work closely with the RN Case Manager to ensure the care of the patient is completed and expectations exceeded. You will effectively interact with all members of the Interdisciplinary Team as well as with your patients and their loved ones while maintaining the highest standards of care and professional nursing clinical competency.
