What are the responsibilities and job description for the Hospice & Palliative Care Nurse Practitioner- PRN position at Tapestry Hospice?
A Hospice & Palliative Care Nurse Practitioner is a member of the Hospice & Palliative Medical Team and delivers care to patients in a variety of settings. The Nurse Practitioner conducts Face to Face encounters with palliative patients and makes recommendations to the Interdisciplinary Team (IDT) regarding patient needs and symptom management.
Responsibilities:
- Conducts face to face visits for palliative patients in all settings. This may include physical examination, interpretation of health data, prescribing medications in accordance with state law, ordering routine and diagnostic studies and collaborating with physicians as appropriate.
- Consults with medical colleagues as needed, communicates about patient care provided, and provides reports as required.
- Assists patients and families in determining goals of medical care.
Requirements:
- Current unrestricted license to practice as a Nurse Practitioner.
- Graduate of an approved Nurse Practitioner program.
- Current CPR certification.
- Minimum of 3 years of palliative care and/or hospice experience.
- Excellent observation, verbal, and written communication skills.
- Valid Driver's License
