Description:
As a Hospice Social Worker your role will be utilized as one of three foundational pillars in the patient's overall plan of care, managing all psychosocial aspects of the hospice journey! Tapestry Hospice is searching for an experienced Hospice Social Worker to provide both emotional and practical support to patients and families facing serious illness or end-of-life issues. This position works closely with members of the Interdisciplinary Team as well as with your patients and their loved ones during the hospice experience and beyond.
Qualifications:
- Licensed as a Social Worker through the Board of Georgia
- Possess effective communication skills (including strong verbal and written)
- Possesses a history of building and influencing patient and family relationships
- Holds a current driver's license, car insurance, CPR certification, and positive driving record
- Hospital, home health, or hospice experience is extremely preferred
- The ideal candidate will live in or around the Gainsville area
Responsibilities:
- Assess patient/family social and emotional factors to estimate their potential to cope with terminal illness and death
- Provide psychosocial counseling and support to the patient/family experiencing emotional, social, and economic conflict
- Participate in Interdisciplinary Team (IDT) meetings to establish, review, and revise the plan of care for each patient, including exchange of information, review of problems, and sharing of professional support
- Maintain the dignity of the dying patient
