Description:
This position will be responsible for providing end-of-life care and hospice services to patients in their home or community setting. The Licensed Practical Nurse (LPN) is responsible for assisting in planning for the patient’s and family’s care under the direction of the RN- Case Manager. The LPN will provide skilled nursing care as directed by the RN Case Manager and as described in the interdisciplinary plan of care and will reinforce appropriate care techniques for patients and families after normal business hours
Requirements:
- Ability to work 12 hour shifts on Friday, Saturday and Sunday on a weekly basis (9 am to 9 pm)
- Current Licensed Practical Nurse License.
- A self-driven individual with a desire to deliver excellent customer service.
- Excellent communication skills, verbal and written.
- Excellent clinical assessment skills.
- Computer skills (EMT, email, and familiarity with health care applications).
- Excellent time management skills.
- Ability to accurately document a patient's current status.
- Knowledge of pain management & symptom control.
- Valid Driver's License and reliable transportation

- Hospice Care
- Critical Care, Surgery or Oncology
