Job Description
The Memory Care Coordinator reports directly to the Resident Services Director (aka. DON) and is responsible for the day-to-day operations of Memory Care in an effort to deliver the highest degree of quality resident services. Incumbent shares in the responsibility for the success of the community through management of the department's day-to-day operations. Directs and coordinates the planning, implementation, and evaluation. Completes assessments and develops care plans to address resident needs and services. Serves as a liaison between to the RSD. Makes efforts to reduce safety hazards, accidents, and injuries and works closely with the RSD and Business Office Director concerning responsibilities in accordance with the organization’s policies and applicable laws including hiring, staff evaluations, merit increases, disciplinary actions, and terminations.
Supervisory Duties: This position supervises Memory Care Caregivers, Med-Technicians, and Program Assistants.
Qualifications
Required Skills/Abilities:
0 Demonstrate an understanding of the aging process and the skills required to effectively communicate with seniors.
0 Experience in the health, human services, and/or community services field.
0 Possess strong organizational and time-management skills and maintain consistent and regular attendance.
0 Must have demonstrated ability to hire, direct, and manage personnel.
0 Remain abreast of current State regulations and Nurse Practice Acts and compliance with regulatory processes, such as State, OSHA, and EPA.
0 Ability to present information and respond to questions from groups of clients, customers, and the public.
0 Must possess a high degree of interpersonal relations skills and capability of relating to various people and personalities. Considerable initiative, judgment, delegation, and leadership skills are also required.
0 Must have excellent oral and written communication skills.
0 Ability to read and comprehend reports, business correspondence, and procedure manuals.
0 Proficient with Microsoft Office, ADP, Electronic Health Records, and the ability to comprehend other related software.
Education and Experience:
0 College degree preferred and First Aid/CPR certified.
0 Experience in senior living in the areas of resident care, engagement programs, or similar areas.
0 Experience working with people with cognitive impairment in a supervisory position is preferred.
0 Must have a valid unrestricted driver’s license and pass a criminal background check as required by state law.
Physical Demands:
0 While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel and talk or hear. The employee frequently is required to stand, walk, and occasionally required to sit; reach with hands and arms; stoop, kneel, crouch, or crawl; and taste or smell.
0 The employee must occasionally lift and/or move up to twenty-five (25) pounds. May need to assist residents in getting in and out of bed, bathroom, chairs, etc. May occasionally need to lift residents with the help of other employees.
0 Able to work some weekends and available by phone 24 hours a day, 7 days a week.

Partners with the Life Enrichment Director to
- Ensure the development and implementation of the monthly life enrichment calendar based on the Memory Care program tenets which include, but are not limited to, type and timing of activities, the three components of active daily living, and the dimensions of wellness.
- Ensure a variety of appropriate activities are available and implemented 24 hours per day and 7 days per week and Memory Care staff are actively involved and engaged with activities.
- Conduct individual resident history, preferences assessments, and develop individualized service and program plans.

Anticipated wage range is $50,000 - $53,000 / year
