Responsible for coordinating home health care services for TARLANI Healthcare patients, which includes establishing and maintaining the patient's plan of care, establishing and maintaining positive relationships with assigned patients.
Essential Job Functions/Responsibilities
- Coordinates home care services for patients assists with the establishment of the plan of care in coordination with the physician and the DON.
- Keeps live contact with the doctors and makes sure that all the updated orders are submitted to doctors in a timely manner.
- Verifies insurance coverage and seeks reauthorization for ongoing and additional services.
- Works with Clinical Supervisors/Nursing Supervisors and staff clinicians to gather, collate, and report patient care summary information to Case Managers.
- Maintains comprehensive working knowledge of contractual relationships and ensures that patients are admitted and services are delivered in accordance with contract provisions.
- Establishes and maintains positive working relationships with current and potential customers.
- Promptly responds to TARLANI Healthcare customers' requests and concerns.
- Assists in building a high-quality provider of services.
- Maintains comprehensive working knowledge in the field and shares information with appropriate organization personnel.
- Other duties as assigned.
Position Qualifications
- Nurse with current licensure to practice professional nursing in the state.
- Graduate of National League for the Nursing accredited school of nursing.
- Must have at least two years experience in home health, with at least one year in home healthcare coordination.
- Complies with acceptable professional standards and practice.
- Demonstrates good communication, negotiation, and public relations skills.
- Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
- Ability to manage 150 - 300 active cases
- Cover on-call every 3rd or 4th week of the month, including weekends.

Pay: $50.00 - $70.00 per hour
