Re-entry Intensive Case Management Community Health Worker (CHW) / Case Manager
Description:
Seeking full-time Community Health Worker or Case Manager for Justice involved re-entry intensive case management program. Candidate will provide connections to direct services; care coordination and system navigation; coaching and social support; education about the health and social service system; advocacy; outreach; assessment; and capacity building. Additionally, candidate will serve as liaison and cultural mediator, continuously educating members of the health and social service system about community strengths and needs, and may participate in research and evaluation about the program.
CHW Job Requirements:
High school diploma or equivalent.
Relevant Work and Lived Experience
- Lived experience, meaning having been incarcerated in the criminal justice system
- Member of the community, close affiliation, or shared life experience with the community being served
- Ability to work appropriately and effectively within the program
- Potential or demonstrated community leadership
- Experience working with medically and socially complex individuals
CM Job Requirements:
BA/BS/BSW degree in social work or related fields
- Experience working with re-entry population
- Bilingual English/Spanish preferred
- Valid California driver’s license and insurance
