What are the responsibilities and job description for the Community Skills Trainer position at TASC, Inc.?
Must read and respond to emails on each shift.
Must maintain accurate hours worked on a daily basis, via on-line time keeping program/system.
Complete and submit all required paperwork timely (including but not limited to daily service documentation, incident/accident/illness reports, transportation logs, proof of purchase for any expenditures).
Shall be knowledgeable in dealing with medical and household emergencies.
Support a warm, home atmosphere in each home (based on individual(s) personal choices and provide informed choices about community activities and cultural event opportunities.
Must have the ability to assist individuals with individuals’ personal cares (i.e. bathing/showering, toileting, grooming, hygiene, shaving), basic cooking skills, cleaning / light housekeeping, laundry, grocery shopping, day to day living activities, exercise, transportation (includes medical appointments if necessary).
Must be able to provide a variety of supports to individuals, including by not limited to verbal, gestural, modeling, hand over hand and physical.
Must carry out individual health care plans and monitor individual use of medications.
Must regularly read individuals program plans and demonstrate the ability to skill build through achievement of stated goals and objectives as outlined in their individual program plans.
Encourage individuals to complete various tasks/activities to achieve their personal goals and objectives.
Demonstrate knowledge of individual histories and service plans and keep up-to-date with their current progress and/or medical health status. This includes monitoring induvial health status or significant behavioral / health changes and communicating observations to supervisor.
Be aware of any factors impacting the individual (i.e.: means of communication, required degree of supervision, guardianship, special needs, medications, Rights limitations (which includes being able to identify when limitations can be lifted) and general health.
Must attend and participate in applicable workshops and seminars to update skills and knowledge. May involve overnight travel.
