What are the responsibilities and job description for the Hospital Liaison position at Taylor Life Center?
The Hospital Liaison is responsible for meeting with persons served in crisis. Services are provided to children, adults, and/or families. The Hospital Liaison will coordinate discharge of persons served by setting follow-up appointments with case managers, therapists, and/or psychiatric clinic as necessary, complete addendums to IPOS, renew and/or review Relapse Prevention/Crisis Plan, along with communication with other personal to ensure person served is in an adequate setting and follow-up care is secured. The Hospital Liaison will carry a case management case load, along with assisting in other duties as necessary.
This position requires traveling in the community to hospitals and/or correctional facilities, assisting persons served with transportation to and from inpatient setting or crisis screening as necessary and deemed safe and appropriate, which may include transporting persons served.
Qualifications and Education Requirements
Applicant(s) must be qualified as a CMHP with 3 years experience with a bachelor's degree or 1 year experience with a master's degree in the examination, evaluation, and treatment of minors and their families. State license (preferred) as an LMSW, LLMSW, LMFT, LLMFT, LBSW, LLBSW, LPC, LLPC, TLLP, LLP, LP, RN, or a degree in a human service field from an accredited school. Human service fields can include, but not limited to Human Services, Psychology, Sociology, Criminal Justice, Social Work, Education, etc.
