Badge photo required within a week of signing. This is due no later than Monday before start. Photo must be in color from mid-shoulders up. Candidate must review and sign within 7 days of signing contract.
Physical within a year of start, and updated annually. Labs must be complete / scheduled within 7 days of signing contract. Must be signed by MD, NP, PA, DC, APN or DO. The physician's credentials, clinic location, and phone number must be listed on the form. Statement must indicate candidate is in good physical and mental health, free of communicable disease, and able to perform the core functions of the position without restrictions.
