Position / Specialty : PTA Outpatient

Shift : 5x8 Days (0800-1630)

9 visits / day. 25 rooms / beds.

Years of experience REQ : Minimum 1 year experience required.

First-timers accepted : Yes.

Weekend REQ : No.

Is on-call REQ? No.

Will clinician float within scope to meet facility needs? No.

Open to accommodating block schedule? Discuss schedule / time off requests with manager.

Locals accepted : Yes- no radius.

Must have active or pending IL license to apply.

Job Requirements

Certs REQ : BLS and state license required.

Special Requirements : Outpatient Clinic experience required.

Experience taking Vital Signs required.

Required Documentation Skills / Experience : Assessment, Discharge, Short Term Goals, Time Management.

Required Professional Skills and Knowledge : Home Exercise Program, Lifting and transfer, Signs & Symptoms physical, sexual, psychological abuse.

Required : PTA and State License.

Schedule Information

Guaranteed Hours : Client may call-off up to 36 hours per thirteen week assignment without paying for the cancelled hours.

If Client cancels more hours than permitted, Client will be billed for any shifts or portions thereof cancelled in excess of this section.

If Candidates are placed on-call in lieu of being called off, that will count as a called-off shift.

Unit Specific Information

Scrub Color : No specific scrub color. And business casual or professional athletic attire.

Additional Details

A legible copy of a valid / current US Driver’s license or US ID card

Candidate must review and sign within 7 days of signing contract.

Physical within a year of start. Labs must be complete / scheduled within 7 days of signing contract.
