We are hiring a Full-Time permanent Case Manager to work onsite in a Hospital setting near Indianapolis, IN near 46222.
Hours are Monday-Friday onsite. Somewhat flexible hours, but generally 8:00 AM – 4:30.
No weekends.
Adult LTAC patients
Responsibilities:
- Coordinate and facilitate patient care with the ICT team.
- Monitor patients throughout their care journey, ensuring optimal resource use and compliance.
- Provide support through assessment, care planning, implementation, and evaluation.
- Enhance patient management and satisfaction by integrating case management, utilization review, and discharge planning.
Qualifications:
- Education: Graduate of an accredited program required:
- RN, BSN preferred; or
- Master of Social Work with licensure as required by state regulations; or
- Bachelor of Social Work with licensure as required by state regulations
- Licenses/Certification: Healthcare professional licensure required as Registered Nurse, or Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW) if required by state regulations. Certification in Case Management preferred.
Experience:
- Two years of experience in an inpatient setting preferred.
- Prefer prior experience in case management, utilization review, or discharge planning.
Essential Functions:
- Care Coordination: Coordinate clinical and psycho-social activities with the Interdisciplinary Team and Physicians.
- Monitor patient stays for effective care coordination and facilitation.
- Stay updated on reimbursement, community resources, case management, psychosocial, and legal issues.
- Refer high-risk patients for additional support.
- Advocate for patients, enhancing collaborative relationships for informed decision-making.
- Provide age-appropriate care based on growth and development principles.
- Participate in interdisciplinary rounds/conferences to review goals, optimize resources, educate families, and identify post-hospital needs.
- Collaborate with clinical staff on care plans and goal achievement.
- Coordinate care planning with the interdisciplinary team, physicians, patients, families, post-acute providers, payors, and others.
Discharge Planning:
- Conduct ongoing patient assessments for timely and safe discharge planning.
- Develop and execute effective discharge plans using critical thinking.
- Coordinate and communicate efficiently with patients and families.
Knowledge/Skills/Abilities/Expectations:
- Knowledge of payor practices, regulations, standards, reimbursement, Medicare benefits, insurance processes, and accreditation standards.
- Critical thinking, prioritization, and time management skills.
- Fluent in English (reading, writing, speaking).
- Basic computer skills, including Microsoft Office.
- Excellent interpersonal, verbal, and written communication skills.
- Good interpersonal skills with patients, families, and staff.
- No travel required.
- Performs other related duties as assigned.
