This role can further serve as a partner to help address social determinants of health concerns and other related wellbeing challenges presented by this company's employees. This role will be a hybrid role at 3 customer locations around Bronx, NY.What you’ll be doing : This role will sit onsite at 3 of the customer’s location around Bronx, NY. The role is Monday-Friday with the hours of 8am-4 : 30pm EST; either Tuesday or Friday will be the Remote day for this role.Identify and make referrals for work-life, financial & legal, and basic needs support.Link employees to appropriate local and national resources via one-on-one consultation and referral list(s).Work with customer internal care teams on behalf of those employees in need of emergency funding and resource referrals.Offer consultation related to navigating mental health resources and available workplace benefits.Assist in actively linking employees to appropriate mental health treatment options and provide support throughout applications and follow-ups as needed.Spring Health benefit promotion : relationship building with customer POCs, tabling events, benefit overviews, rounding, wellness events, etc.Electronically document and track metrics / outcomes.Represent Spring Health and help to support / reinforce program parameters and goals.Communicate with local points of contact and cascade any needs to Spring Health partners.Submit reporting documentation of all client and organizational activities on a routine and timely basis.Attend customer meetings as requested / appropriate.Embed in the customer culture and proactively build strong working relationships with key customer stakeholders.Develop trusting relationships with team members by providing support and advocacy to help achieve health and wellbeing goals.Demonstrate cultural humility and promotion of diversity, equity and inclusion.What we expect from you : A Bachelor's degree in a similar field is required.Bilingual in Spanish and English required.Experience working with diverse populations and backgrounds within the social work industry.Experience connecting individuals to resources quickly in times of crisis is a plus.Ability to balance the needs of multiple stakeholders.Flexibility and creativity when approaching workplace challenges.Excellent listening, written and verbal communication skills.Motivated to go above and beyond for members to provide a high-touch experience.Tech-savvy : must be comfortable using various computer platforms and learning new systems.Stable transportation and ability to work from multiple onsite locations.Located within a 10 mile radius of job location listed.The target salary range for this position is $63,000 - $80,000, and is part of a competitive total rewards package including stock options, benefits, and incentive pay for eligible roles.Ready to do the most impactful work of your life?
