We are bold builders, humble learners, and focused on urgency, committed to our mission to tirelessly pursue results.If this vision resonates with you, we hope you consider bringing your creativity, energy, and curiosity to Waymark.About this roleAs an Outreach Specialist / Care Coordinator, you will serve as Waymark’s primary liaison with healthcare providers and patients, helping to shape and refine our workflows and build strong relationships with our patients and primary care physician partners. You will work to improve the health of patients from low-income communities by fostering trust and connecting them with various providers and resources.ResponsibilitiesConduct daily outreach to eligible individuals by phone and SMS to establish relationships and provide support.Achieve individual patient engagement goals through effective and empathetic communication.Support patients by helping them navigate healthcare and social resources.Review patient data and healthcare utilization information to assist with care coordination.Build trusted relationships with healthcare provider partners and community organizations.Communicate with primary care teams and other providers to coordinate patient care.Participate in weekly care team huddles and support coordinated patient care.Document patient outreach and encounters accurately in technology systems.Collaborate with the support team to refine workflows for patient care.This is a hybrid role, involving mainly remote work and occasional in-person meetings.Passion for working with diverse communities in an empathetic and professional manner.Friendly and energetic personality with a patient service orientation.Experience with telephonic outreach or customer service roles, preferably in healthcare.Excellent verbal and written communication skills.Highly organized and self-motivated to manage schedules efficiently.Strong computer skills with experience using technology for communication and documentation.Comfortable with ambiguity and energized by a fast-paced startup environment.Reside within the greater Seattle or Snohomish County areas.Preferred qualificationsExperience in a healthcare delivery setting.Experience with care coordination or working in an outpatient clinic.Experience with healthcare data and / or Electronic Health Records.Knowledge of managed care or health insurance.Knowledge of the Greater Seattle or Snohomish County communities and Medicaid populations.Salary Range : $23.50 / hour - $30.40 / hour.
