What are the responsibilities and job description for the Pediatric Home Health Nurse position at Team Select Home Care?
Team Select Home Care is looking for a dedicated and compassionate Home Health Nurse to join our growing pediatric care team in Glendale, AZ. As a Home Health Nurse, you’ll provide one-on-one care in a home setting, helping improve the quality of life for our pediatric patients.
Job Type: Full-Time Tuesday – Friday, 6:00 AM – 4:00 PM
Pay Rate: LPN $29-$31/hour RN $35-$39 hourly
Responsibilities:
- Deliver high-quality nursing care to pediatric patients in their home environment
- Monitor and assess patients’ health conditions, including vital signs, and report any changes to the supervising physician
- Administer medications, treatments, and other prescribed interventions as directed
- Maintain accurate and timely documentation of patient care and progress
- Provide support and education to families and caregivers
- Collaborate with physicians and other healthcare professionals to ensure optimal care
Requirements
- ✅ Valid LPN or RN nursing license (State of Arizona)
- ✅ Current CPR certification
- ✅ Experience with G-tube (g-button) care is required
- ✅ Strong clinical skills with a focus on pediatric care
- ✅ Reliable transportation and willingness to travel to patients’ homes within the designated service area
