Who We Are Looking For:
An Assertive Community Treatment (ACT) team consists of a community-based group of medical, behavioral health and rehabilitation professionals who use a team approach to meet the needs of an individual with severe and persistent mental illness. An ACT Case Manager spends the majority of their time in the community.

You will collaborate with seasoned professionals on cases ranging from depression to severe and persistent mental illness, as well as drug addictions. You will have a direct impact on the quality of member care. Our outpatient clinics treat more than just the person’s illness. You will have the chance to use your knowledge and care coordinating social work skills to provide assistance with linking members to insurance, housing, food, employment, personal care, community integration and family reunification. Position requires you to drive members in your personal vehicle to appointments. You will be part of a well-round team and linked directly to the member’s overall treatment plan; to ensure that they are safe, socially thriving, and living a fulfilling life of recovery, in the least restrictive environment.
