Qualifications:
- High School Diploma or GED required
- Must be Certified Peer Support Specialist
- Must have a valid Driver’s License with a driving record that is acceptable to the company’s insurance providers
Responsibilities:
- Screen and evaluate member for potential eligibility for assistance programs
- Complete documentation related to all indirect and direct services collaboratively with member
- Act as an advocate on behalf of members in order to ensure that all appropriate services and/or referrals to services are made available
- Conduct appeals as necessary to advocate on behalf of member to receive benefits and entitlements
- Facilitate member participation in treatment related services.
- Assist members with completion of group related activities.
- Provide services specific to assisting members with housing and/or vocational activities
- Provide member transport when necessary with valid driver’s license and active auto insurance
- Promote and support member recovery by conducting education activities for the person served, as applicable: self-advocacy, wellness, life skills goal setting, decision-making skills
- All other duties and responsibilities as assigned by the Clinical Services Supervisor
