About the Role :
- Provide comprehensive primary care services 3 days per week
- Collaborative with interdisciplinary teams to optimize patient outcomes
- Ensure smooth transitions of care with hospice, palliative care, and home care agencies
- Maintain accurate and timely documentation in our EMR system (Gehrimed)
Qualifications :
Clinicians trained or practicing medicine in the following disciplines are encouraged to apply : family, primary, internal, adult, post-acute, home health, hospice, and hospital medicine. New grads are welcome!
Our compensation is a productivity-based model (Fee for Service). Expected compensation is estimated range of $54k - $88k annually with no cap on productivity income potential.
