We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team in East Greenbush. As a post-acute care clinician, you will work closely with interdisciplinary teams to provide high-quality, safe, and cost-effective patient care. Your responsibilities will include collaborating with other clinicians, working with hospice and palliative care agencies, and ensuring timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines.
Required Skills and Qualifications
To be successful in this role, you will need to possess excellent communication and clinical skills. You should be comfortable working in a fast-paced environment and be able to prioritize multiple tasks simultaneously. A recent graduate or experienced candidate with a willingness to train is welcome to apply.
