Responsibilities
- Deliver primary care services in post-acute care facilities, including skilled nursing and assisted living.
- Drive high-quality, safe, and cost-effective patient care.
- Collaborate with interdisciplinary teams and adhere to CMS guidelines.
- Work closely with hospice, palliative care, and home care agencies for seamless patient transitions.
- Provide oversight, guidance, and mentorship to clinicians within facilities.
Opportunity Overview
- Full-time schedule: Monday through Friday.
- Patient volume: 15-20 patients per day.
- Compensation: Our productivity-based model (fee for service) allows for maximum income potential. Expected compensation ranges from $171,000 to $277,000 annually.
Qualifications
- PREFERRED: Physicians with internal medicine or family medicine backgrounds.
- We welcome physicians with other backgrounds who are passionate about post-acute care and new graduates.
