San Angelo, the county seat of Tom Green County, is located in the heart of West Texas at the juncture of the Middle and North Concho Rivers. Role Overview
- Schedule: have scheduled patients 1 day a week
- Provide patient care in a skilled nursing facility by conducting rounds in their short-term units
- Conduct documentation at home with provided iPad laptop within 48 hours of seeing patient
- Perform assessments and therapy sessions
- Collaborate with facility staff to enhance patient care
- Ensure timely and accurate documentation
- Hold a valid full license as either LPC or LMHC
- Experience in healthcare or long-term care is preferred but not required
