About The Role
You will play a vital role in delivering compassionate and effective care to our patients in post-acute care communities. Your responsibilities include:
- Providing comprehensive primary care services to a manageable patient panel of approximately 15 patients per day
- Collaborating with interdisciplinary teams to optimize patient outcomes
- Ensuring smooth transitions of care with hospice, palliative care, and home care agencies
- Maintaining accurate and timely documentation in our EMR system (Gehrimed)
