Job Description

Join our post-acute care team to provide care to patients / residents in need of rehabilitative care in our skilled nursing facility.
Responsibilities :
- Provide oversight and mentorship to our clinicians in the greater Columbia, South Carolina, area
- Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care
- Represent TeamHealth in engaging, establishing, and maintaining relationships within the community
- Provide clear communication with clinicians, facility leaders, and staff regarding care and services for enrollees
- Collaborate with leadership to review care coordination initiatives
- Develop intervention plans, and share ideas about network management issues
- Work alongside pharmacists on medication management
- Complete Medicare required visits
- Complete acute, follow-up, and discharge visits as necessary
- Implement an individualized plan of care for patients' goals
- Make recommendations on transitional care, DME, home health services, or outpatient services after a short stay
- Facilitate and assist leadership in quality improvement initiatives and risk management
- Timely response in chart completions and pharmacy recommendations
Experience as a medical director in a skilled nursing facility preferred. However, clinicians trained or practicing medicine in the following disciplines are encouraged to apply : family medicine (FM), primary, internal medicine (IM), adult, post-acute, home health, hospice, palliative, and hospital medicine (HM).
Our compensation is a productivity-based model (Fee for Service). Expected compensation is estimated to range from $177,000 to $225,000 annually with no cap on productivity income potential. Click apply to learn more!
