What are the responsibilities and job description for the Home Health RN position at Tech Medical Home Care Services?
PURPOSE:
To establish a job description for a Registered Nurse (RN) in a Skilled Nursing Services Agency . The Staff Nurse is an experienced professional who administers skilled nursing care to the patients on an intermittent basis in the home. He/She independently asses plans and delivers this care in accordance with the physician's orders.
POLICY:
Responsibilities
1. General observation and evaluation of a patient's skilled needs. Complete medical history of patient, complete physical assessment which includes all systems, a socioeconomic assessment, and essential identifying information of the client
2. Establish a Care Plan based on: the Physician Plan of Treatments, RN Findings of initial assessment and patient and/or caregiver input
3. Identify additional services as order by the Plan of Care
4. Instruct patients and/or caregivers on how to make optimal use of available resources
5. Communicate with the physician as the patient's condition requires
6. Communicate with other professional workers sharing pertinent information with appropriate agencies as patient's condition indicates.
7. Resolves patient problems and needs by utilizing multidisciplinary team strategies
8. Maintain necessary records and reports with appropriate documentation
9. Prepares clinical and progress notes
10. Instruct patient and/or caregivers regarding patient care and guiding them towards independence
11. Promotes patient's independence by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills; answering questions
12. Perform and compete supervisory visits as follows: All skilled clients require a supervisory visit every 14 days (HHA does not need to be present). All Non-skilled clients require a supervisory visit on their HHA every 60 days (HHA to be present)
13. Provide for patient's medical and physical safety under the Plan of Care in the home
14. Adhere to all policies and procedures as set by Tech Medical Inc.
15 . Protects patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.
16. Perform other duties as assigned by DON (ex. OASIS Assessments)
17. Actively participates in nursing organizations and promotes and participates in in-service programs
18. Adheres to the policies and procedures as defined in the Policy and Procedure Manual and the Employee Handbook
Job Conditions
1. Position is stressful in terms of responsibilities and meeting deadlines
2. The ability to drive and the ability to access client's homes which may not be routinely wheelchair accessible
3. Hearing, eyesight and physical dexterity must be sufficient to perform physical assessment of the client's condition and to perform client care
4. On occasion it may be required to bend, stoop, reach and move client weight up to 250 pounds; lift and or carry up to 30 pounds
5. Must be able to communicate clearly, both verbally and in writing
6. Risk of blood borne pathogens is limited
Equipment Operation
1. Requires the ability to utilize a PC, calculator, multi-line telephone, and other office related equipment.

1. Has access to all client medical records which can be discussed with all management staff , including Administrator and Director of Nursing.
Qualifications
1. Shall be a licensed Registered Nurse in the State where work will be performed.
2. Shall be a healthcare professional with training and experience in health services and at least 1 year supervisory experience in home health care or related health programs
3. * Must meet initial and ongoing educational requirements as stipulated by local, state and federal guidelines.
