Third Shift REMOTE Care Coordinator - working directly with the Concierge Team- After Hours, holidays and Weekends.

HOURS : 5pm – 8am during the week, and 24 / 7 on weekends and holidays

Remote – Florida based

Position Purpose :

Supports care management activities and the teams assigned to members to ensure services are delivered by the healthcare providers and partners and continuity of care / member satisfaction is achieved. Interacts with members by performing member outreach telephonically or through home-visits and documents the plan for care / services of activities.

Provides outreach to members via phone or home visits to engage members and discuss care plan / service plan including next steps resources questions or concerns related to recommended care and ongoing education for the member throughout care / service as appropriate

  • Taking calls from parents and caregivers anytime the caregiver is experiencing any unexpected issue in the care of the child (night nurse called off, medication not being filled at pharmacy, delivery of medical supplies delayed, etc.)
  • Taking the information provided from the call and working with the caregiver / parent to find resolution.
  • Supporting 800 – 1000 members (small population)
  • Calls are based on volume.
  • Most important note would be that the team is looking to deliver quality care to each caregiver / parent
  • White Glove Service
  • All training will be held online

REQUIREMENTS NEEDED :

Call center experience

healthcare experience

Front desk / receptionist within a healthcare setting

customer service within a healthcare setting

1. 2-3 years managed care setting, medical office or facility setting with demonstration of medical administration duties

2. Thorough knowledge of customer service, utilization review or claims processing practices in a managed care environment

3. Troubleshooting and problem solving

Education / Experience :

Requires a High School diploma or GED

Requires 1 - 2 years of related experience

Pay and Benefits

The pay range for this position is $18.00 - $22.00 / hr.
