JOB STATUS: Full time, non-exempt
SUPERVISION RECEIVED AND EXERCISED: The employee in this position reports to the Aging and Disability Resource Center Director; the employee exercises no supervision.

- Accept referrals from the Elder Helpline as well as other resources for initial intake and screening.
- Informs clients about Aging and Disability Resource Center programs and program requirements, obtains background information, and initiates the process to inform, guide, direct, and link elderly clients to appropriate resources and services.
- Determine the individual’s needs and screen for potential eligibility for Medicaid-related and state/federal funded services utilizing a standardized screening instrument.
- Provide efficient, timely and consumer friendly services to facilitate the screening and review process.
- Provide information on service options, service providers, and how the services will be provided, identifying alternative options when client is not eligible for publicly funded services.
- Provide crisis intervention to stabilize family situation and identify relevant community supportive services.
- Conduct comprehensive screening on Medicaid probable individuals on the Assessed Priority Consumer List (APCL) who are not active in a case managed program.
- Input client assessments into eCIRTS (Client Information and Registration Tracking System)
- Post information in the enrollment and termination screens of eCIRTS to update the APCL as needed.
- Contact individuals on the APCL as required to update information and screen for Medicaid eligibility and state/federal funded program.
- Ensure that all client data is transmitted, stored, and disposed of according to the confidentiality requirements of Northwest Florida Area Agency on Aging and all federal and state regulations.
- Attend conferences, workshops, and meetings to keep abreast of developments and issues related to the functions of this position.
- Perform other duties as assigned, which may include counseling disaster victims about Medicaid programs, options, and the eligibility process.
- Employee may be required to work specific hours, days, or holidays to provide adequate coverage for clients.
PHYSICAL REQUIREMENT OF THE POSITION: Employee must be able to maintain effective audio-visual discrimination and perception as needed to make observations, read and write, make practical mathematical computations, communicate with others, and operate assigned office equipment. Employees must maintain mental capacity which permits him/her to make sound decisions, use good judgement, adjust to changing circumstances and emergencies, demonstrate intellectual capabilities and work effectively with others, specifically senior adults. Employees must maintain physical condition appropriate to the performance of assigned duties and responsibilities which may include the following: sitting or standing for extended periods of time and operating assigned office equipment such as telephone, computer, calculator, copy and/or fax machines.
QUALIFICATIONS:
- Have a bachelor’s degree from an accredited college or university in a human service-related field; or
- Have an associate’s degree from an accredited entity in a human service-related field and a minimum of two years’ experience as a caseworker, case manager, intake specialist, or related work experience with the long-term care client population; or
- Have a high school diploma or GED and four years’ experience as a case worker, case manager, intake specialist, or related work experience with the long-term care client population.
LICENSURE OR CERTIFICATION: Possession of a valid Florida driver’s license
SALARY RANGE: $31,200/yr. - $41,600/yr. ----- ($15/hr. - $20/hr.)
