Position Summary
The Nurse Practitioner (NP) at Telehealth Evals LLC is responsible for conducting virtual medical evaluations, diagnosing and treating patients, and ensuring compliance with regulatory standards in a telehealth environment. The NP will collaborate with interdisciplinary teams, including physicians, physician assistants, therapists, and assistive technology professionals, to deliver high-quality care and optimize patient outcomes.
Essential Duties & Responsibilities
Clinical Evaluations & Patient Care
- Conduct virtual patient consultations to assess medical conditions, review histories, and provide evidence-based care.
- Evaluate functional mobility needs and determine the medical necessity for durable medical equipment (DME), mobility assistive devices, and rehabilitation services.
- Diagnose and manage chronic and acute conditions within the NP's scope of practice, utilizing telehealth best practices.
- Document clinical findings, treatment plans, and medical recommendations accurately in the electronic health record (EHR) system.
Patient Education & Coordination of Care
- Educate patients on their diagnoses, treatment options, medication management, and assistive devices.
- Guide patients on preventative care, chronic disease management, and rehabilitation strategies.
- Coordinate care with primary care providers, specialists, and therapists to ensure seamless patient management.
- Address patient questions and concerns regarding their health conditions and recommended interventions.
Compliance & Telehealth Standards
- Ensure compliance with HIPAA, CMS, and state regulations governing telehealth services.
- Stay updated on telehealth best practices, coding changes, and regulatory updates related to virtual care.
- Maintain high standards of clinical documentation and patient confidentiality.
- Participate in internal audits and quality improvement initiatives to enhance telehealth services.
Interdisciplinary Collaboration
- Work closely with physicians, physician assistants, occupational therapists, physical therapists, and assistive technology professionals (ATPs) to develop comprehensive treatment plans.
- Collaborate with billing and compliance teams to ensure proper coding and reimbursement for services.
- Participate in team meetings, clinical case discussions, and professional development sessions.
Qualifications & Requirements
Education & Certification
- Required: Master’s or Doctorate degree in Nursing from an accredited program.
- Licensure: Active Nurse Practitioner (NP) license in the state(s) of practice. Licensed in any or all of the following states NJ, PA, GA, FL preferred
- Certification: Board-certified as a Family Nurse Practitioner (FNP), Adult-Gerontology NP (AGNP), or equivalent.
- Preferred: Certification in telemedicine, functional mobility assessments, or chronic disease management.
Experience
- Minimum of 2 years of experience in primary care, physical medicine & rehabilitation, geriatrics, or chronic disease management.
- Experience with telehealth patient evaluations and remote care delivery preferred.
- Familiarity with Medicare, Medicaid, and private insurance requirements for virtual healthcare services.
Skills & Competencies
- Proficiency in conducting virtual patient consultations and assessments.
- Strong clinical judgment and diagnostic skills in a telehealth setting.
- Excellent communication and patient education skills to engage patients remotely.
- Ability to work independently while collaborating with interdisciplinary teams.
- Experience with electronic health records (EHR) systems and telehealth platforms.

- Evaluation Accuracy: Maintain ≥ 98% accuracy in assessments and documentation.
- Patient Satisfaction: Ensure high patient engagement and positive feedback in virtual evaluations.
- Claim Approval Rate: Maintain compliance with medical necessity guidelines to minimize claim denials.
- Training Participation: Engage in ongoing telehealth education and professional development.
Work Environment & Expectations
- This is a remote position requiring a professional and secure workspace with reliable internet access.
- Flexible scheduling may be required to accommodate patients across multiple time zones.

Job Type: Full-time
Pay: $122,000.00 - $180,000.00 per year
