Compensation: $72K- $88K - SIGN ON BONUS!
The Extended Care RN provides seamless continuity of nursing care between routine business hours and after hours by triaging phone calls, providing follow-up visits for symptom management, ordering supplies and equipment, educating patients, families, and caregiver, attending death visits, and extending support to families and caregivers at the time of death. They also conduct hospice assessments and admissions during the evenings and weekends. Excellent communication between the Extended Care RN and the rest of the Interdisciplinary Team ensures consistent care is provided to patients.

Qualifications/ Experience
- Valid Registered Nurse license in the State of Michigan and ability to receive credentialing at partner hospital(s).
- BSN or master’s degree preferred. Associate degree/diploma.
- 2 years of nursing experience, 1 year experience in acute care unit or hospice experience preferred.
- Experience in Electronic Medical Records.
- Collaborates with answering services to provide triage for patient and family needs after hours by utilizing Extended Care workflow guidelines.
- Coordinates and leads other Hospice weekend staff to meet productivity and patient/family/caregiver needs.
- Provides initial and ongoing assessments of physical, psychological, social, and spiritual needs of the patient/family/primary caregiver and initiates appropriate intervention or referral.
- Coordinates patient care, regularly reviews and updates interdisciplinary care plans, communicates with the team, and submits the required documentation in a timely manner.
- Conducts scheduled and urgent information/sign-ons and admission assessments for Hospice referrals and ensures paperwork and documentation is completed accurately and timely.
- Creates and participates in non-admission follow-up plan of care and communicates with referral sources to provide appropriate alternatives for patients who do not meet hospice criteria.
- Assists with providing bereavement counseling to the family and/or primary care giver as determined by the Interdisciplinary Team.
- Provides skilled nursing care, such as wound dressings, IVs, blood draws, and catheter insertion and care.
- Actively engages and communicates with the patient’s Primary Care Physician as well as the Hospice Medical Director to ensure exemplary end-of-life care.
- Educates patient, family, and caregivers about disease, signs and symptoms of dying, symptom management and medication management.
- Evaluates, orders, and oversees medication, medical supplies and durable medical equipment ordering to meet patient and organizational needs.
- Reinforces the work of other members of the Interdisciplinary Team, including assisting Hospice Aide team by providing personal cares when needed.
- Performs death visits, including pronouncing patient at time of death, delivering post-mortem care, and providing emotional and spiritual support to family or caregivers present.
- Maintains compliance with Hospice policies and procedures, as well as Federal, State, and local laws and regulations.
- Participates in providing education to interdisciplinary team members regarding discipline-specific aspects of patient/family care.
- Part of clinical team that delivers clinical services 24 hours/day, 7 days/week. This includes a rotation of holidays.
- Works in a variety of environments, including patient homes and healthcare facilities. Frequent travel required within the service area of Hospice Center.
